Covid-19 Agreement *

As a result of COVID-19 public health crisis, J5 NYC dba J5 Fitness has put into place a number of measures to ensure the safety and health of its staff and agents, the public and individuals seeking to participate in J5 NYC dba J5 Fitness’ fitness and health activities as well as to help control the spread of COVID-19. By participating in J5 NYC dba J5 Fitness, I make the following representations and additionally agree to follow the Department of Health's and the J5 NYC dba J5 Fitness’ policies and procedures to reduce the spread of COVID-19 as follows:

I am not experiencing any symptoms of illness such as a fever, cough, or shortness of breath in the last 14 days proceeding session or class date and I have not come into any known contact with a person confirmed or suspected to have COVID-19 in the past 14 days of the session or class. If I develop these symptoms, I agree that I will refrain from attending any in-person fitness or health activities (including, but not limited to fitness training, boxing, kickboxing, cupping therapy and other related) with J5 NYC, LLC dba J5 FITNESS.

If I begin to experience any symptoms of COVID-19 while participating, I will inform J5 NYC dba J5 Fitness and immediately end my participation. I agree to inform the J5 NYC dba J5 Fitness if I have been diagnosed with COVID-19 before or after participating in J5 NYC dba J5 Fitness’ fitness and health activities.

I have not traveled internationally in the last 14 days of session or class date. I do not believe that I have been exposed to a person with a confirmed or suspected case of COVID-19.

I am following recommended guidelines as much as possible - practicing social distancing by participating in group activities of fewer than 10, trying to maintain separation of six feet from others, wearing a mask, and otherwise limiting my exposure to the coronavirus.

I am aware that I must follow the safety and hygiene protocols that have been implemented by J5 NYC, LLC dba J5 FITNESS. Upon arrival for any session or class, I will sanitize my hands by using a hand sanitizer or equivalent provided by the J5 NYC dba J5 Fitness. I will wash my hands with soap and water for a minimum of 20 seconds, and will maintain good hand hygiene throughout my participation after using the restroom, sneezing, or coughing, and before and after touching any common surfaces. I will wear clean workout clothes and wipe down any equipment used with provided cleaner.

I acknowledge and fully understand that I will be engaging in fitness and health activities, including, but not limited to, fitness training, boxing, kickboxing, cupping therapy and other movement based activities that involve risk of damage to personal property or serious injury, including permanent disability and death, and severe social and economic losses which might result not only from the my own actions, inactions or negligence, but the actions, inactions or negligence of others, the condition of the premises or of any equipment, including the exposure to people with infectious diseases, fitness equipment used, contact with others, uneven ground and hazardous materials, and the effects of weather, including heat, humidity, cold and/or precipitation. Further, there may be other risks not known or not reasonably foreseeable at this time. I understand and agree that the J5 NYC dba J5 Fitness is not responsible for any potential exposure to COVID-19 or the consequences therefrom.

I assume all of the foregoing risks and accept personal responsibility for all expenses, medical or otherwise, following any such damages, injury, permanent disability, or death to me or the minor participant. The 'Covid 19 Agreement' form may not be modified in any way. If any part of this waiver is determined to be invalid by law, all other parts of this waiver shall remain valid and enforceable. By filling out this Agreement, you are agreeing that you have read and understand J5 NYC, LLC dba J5 Fitness’ 'Covid-19 Agreement.'
